Open that link in Safari, tap the Share button, then choose Add to Home Screen. Safari specifically — iOS will not let other browsers install it properly.
Android
Open that link in Chrome. It will offer to install it for you. If it does not, the three-dot menu has Add to Home screen.
Already added it before 26 September? Delete the tile and add it again. Earlier installs had no way to update themselves, so they are stuck on whatever version they were added at. This one keeps itself current from now on. The version you are running is at the bottom of the Guide tab.
Where your information goes
Nowhere. Everything you enter is stored on your phone and nothing is sent to us. There is no login and no account, and we never see what you have put in.
The trade is that clearing your browser data clears the tracker too. The Guide tab has a Back up button that saves a copy if you want one.
